Job summary

HealthFinders MN seeks a Youth and Family Engagement Specialist to provide and support direct services to youth and families, aiming to promote health, wellness, and academic success in our service area. Qualifications

• Latino or Somali cross-cultural and linguistic fluency
• Strong customer service skills and ability to maintain confidentiality
• Ability to work some evenings and weekends
• Pass a thorough background check with Department of Human Services
• Educational and community organizing experience preferred but not required

Responsibilities

• Provide direct services to youth and families to promote health, wellness, and academic success
• Lead and facilitate group sessions based on evidence-based curricula addressing social determinants of health
• Collaborate with school-based health center teams and community partners to design and update youth programming
• Provide referrals, care coordination, and follow-up services to ensure student wellbeing
• Maintain confidential records in compliance with HIPAA and state laws
• Support outreach, engagement events, and collaboration with Community Engagement team
• Maintain program records, attendance, and data entry for reporting and evaluation
