Coleman Health Services in Lima, Ohio, is seeking a MRSS Case Manager to serve Allen, Auglaize, and Hardin Counties from our Lima base. You will provide short-term case management and wraparound services to youth and families, coordinating with treatment teams for smooth transitions.

The role requires a bachelor's degree in a social service field or 3 years of related experience, plus willingness to obtain an NPI and Medicaid Billing number with our help.

Competenze

• Bachelor's degree in a social service-related field or 3 years' experience in a similar position.
• Willing to get NPI and Medicaid Billing number (we'll help!).

Mansioni

• Provide short-term case management and wraparound services to youth and families aged 20 and under.
• Transition clients and families into ongoing care.
• Work collaboratively with treatment teams internally and externally to ensure safe transitions.
