Job summary

Damar Services, Inc. in Indianapolis seeks a Community Based Case Manager Supervisor to oversee case managers and ensure compliant records in our EMR. This role carries professional and administrative responsibilities across youth-focused services in our Community-Based program.

You will supervise staff, coordinate with families and schools, maintain documentation, and support onboarding and training to meet DCS and partner standards. A driver’s license and background checks are required.

Qualifications

  • High school diploma or GED required.
  • Requires 2-3 years of experience working with children or youth with behavioral needs.
  • Experience working in an educational setting preferred.
  • Teacher assistant experience or a teaching license is a plus but not required.
  • Proficiency with MS Office applications including: Word, Excel, Outlook and PowerPoint.
  • Minimum age of 21 per licensure requirements.
  • Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving).
  • Be clear of any felony convictions, substantiated charges of neglect or other disqualifying criminal convictions.
  • Successfully complete a pre-employment drug screen and physical.
  • Favorably meet and maintain Damar’s background screening.

Responsibilities

  • Builds and maintains positive, professional relationships with youth to promote engagement, trust, accountability, and progress toward individualized goals
  • Provides direct supervision, guidance, and support to youth while maintaining a safe, structured, and supportive program environment
  • Responds appropriately to youth behaviors and individual needs using approved strategies, interventions, and de-escalation techniques
  • Collaborates with families, schools, referral sources, and community partners to support youth progress and continuity of services
  • Maintains consistent, weekly communication with the school of origin (if applicable), probation officer, family case manager, or other referral source, as well as the parents/caregivers of the youths referred to the program
  • Facilitates daily evidence-based group psychoeducational classes
  • Implements individualized educational continuation plans to support academic progress during program participation
  • Facilitates enrollment and participation in credit recovery and other computer-based educational programs, as appropriate
  • Facilitates community service opportunities and community-based learning activities, when applicable
  • Travels occasionally to support community-based activities, outings, meetings, and other program needs, as required
  • Operates Damar vehicles and/or personal vehicles to transport youth as needed
  • Maintains accurate records, progress notes, educational plans, and program documentation in accordance with agency, contractual, and regulatory requirements
  • Monitors youth progress and adjusts educational, vocational, and transition plans as needed to promote positive outcomes
  • Adheres to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual
  • Completes and updates mandatory training and other licensure-related items in a timely manner
  • Prioritizes duties in a manner consistent with organizational objectives and growth
  • Respects the culture, diversity, and rights of all clients, their families and the community
  • Performs other duties as assigned

Join us in empowering individuals with intellectual and developmental disabilities or autism. Damar is looking for a talented and motivated Community Based Case Manager Supervisor to join our team. The Case Manager Supervisor maintains a continuum of professional and administrative responsibilities related to the oversight and supervision of case managers and their respective cases. The Case Manager Supervisor oversees the interventions and case management services provided for every child and family served in our Community-Based program. This includes direct oversight of electronic and paper records, reports and documentation completed timely in our EMR, as well as processes and systems established to maintain the organization’s full compliance with regulatory standards of care. The Case Manager Supervisor may also facilitate onboarding of new providers as well as ongoing training and support to ensure providers are properly trained and up to date with DCS and stakeholder requirements.

Essential Job Functions

The essential functions of this position require the physical ability and mobility needed to perform the job. This includes the ability to stand or sit for prolonged periods; occasionally stoop, bend, kneel, crouch, reach, and twist; lift, carry, push, and/or pull up to 50 lbs.; operate office equipment, including a computer keyboard; travel to other locations using private or commercial transportation; and communicate effectively to exchange information.
