Job summary

Middle Flint Health & Wellness is seeking a Community Support Individual (CSI) for the APEX program in Sumter County, GA. The role focuses on quality case management services and crisis intervention in schools, homes, and the community to support youth with behavioral health needs.

You will act as a positive role model, coordinate services with agencies, monitor treatment progress, and assist with transportation and documentation.

Qualifications

• High School Diploma or GED required.
• Valid, unrestricted Georgia driver’s license.
• Must be computer literate.

Responsibilities

• Provide quality case management and crisis intervention for the APEX program.
• Act as a positive role model in school, home, and community.
• Identify services and coordinate with agencies to meet youth needs.
• Monitor treatment progress and assist with transportation to appointments.
• Maintain timely documentation and support treatment team with plan of care.
