Youth Advocate Programs, Inc. is seeking a full-time Case Manager to provide comprehensive case management services to individuals and families in need in Los Angeles, CA area.

The role involves assessing needs, developing service plans, coordinating resources, and supporting participants through implementation while ensuring documentation and compliance. Must have relevant education and strong interpersonal skills; bilingual a plus.

Qualifications

• Master’s degree preferred; Bachelor’s degree in social work, psychology, or related field.
• Experience in community-based agency programs.
• Proven experience in case management or similar role.
• Strong knowledge of social issues and community resources.
• Excellent interpersonal and communication skills.
• Organized with good time management.
• Proficient in Microsoft Office 365.
• Knowledge of Electronic Health Record (EHR) systems is a plus.
• Bilingual Spanish speaking is a plus.
• Reliable transportation with valid driver’s license and insurance.

Responsibilities

• Provide intensive case management services to individuals and families.
• Assess participant needs and develop individualized service plans.
• Coordinate connections to appropriate resources and supports.
• Work closely with participants to implement plans.
• Maintain accurate records and documentation.
