Job summary

Unison Health in Ohio is seeking a Case Manager to support youth and families through health interventions for clients with severe and persistent mental illness. You will deliver therapeutic services, coordinate transportation, and document care in the electronic record to help reduce symptoms and restore functioning.

The role requires teamwork, adherence to policies, and compassionate engagement with clients.

Qualifications

Bachelor’s degree in related field preferred or 3 years' experience with youth in similar field.
Knowledge of mental illness including the SED population and crisis management.
Ability to define problems, analyze data, establish facts, draw valid conclusions, and make recommendations.
Positive attitude, commitment to provide superior services and enthusiastic support for the agency mission.

Responsibilities

Provides Therapeutic Behavioral Services to an assigned caseload in accordance with state mandates and agency policies.
Provides transportation to and from various locations in support of the program.
Documents all services provided and maintains records in the electronic record.
Participates in team meetings, supervision, in-service training, and conferences.
Maintains client confidentiality and ensures release of information follows federal guidelines and agency policy.
