The Multi-Agency Alliance for Children is seeking a Youth and Family Coordinator for the PACT program in Columbus, GA. You will maintain a caseload and coordinate the full support process for youth and families, developing goal-oriented action plans and safety plans for crisis situations.

Based within 50 miles of Columbus, GA, you will travel statewide to interact with youth and families, and collaborate with partner agencies while documenting activities and ensuring timely records.

Qualifications

  • Bachelor's degree and 2 years' relevant experience required.
  • Knowledge of Georgia's behavioral health, developmental disabilities, child welfare, education, juvenile justice systems and available resources.
  • Ability to document activities and keep accurate records that are timely and well written.
  • Ability to work effectively on multiple teams with different goals toward the best interest of the youth served.
  • Respect for the value of youth voice, and skill in appropriately involving youth in their care plan.

Responsibilities

  • Statewide travel to agencies, foster homes, and birth homes to meet and interact with youth and families.
  • Coordinate ongoing meetings with youth, families, natural supports, partner agencies and community members (such as DJJ, DFCS, and school systems).
  • Attend and participate in MAAC's Clinical Team Meetings, when necessary.
  • Write and develop plans to achieve youth and family success.
  • Maintaining documentation on a timely basis.
  • Develop resources to meet the unique needs of the youth and families served and make referrals when necessary.

About Us:

The Multi-Agency Alliance for Children is a non-profit organization committed to creating positive outcomes for vulnerable youth and families. Our mission is to coordinate resources, information, and a network of quality providers. We are currently seeking a PACT Coordinator, who will play a pivotal role in working with young people to improve their success and sustainability in a community environment.

Job Purpose:

The YFC maintains a caseload and is responsible for coordinating and facilitating the entire support process for each of the youth and families served. This entails assisting teams with developing individual goal-oriented action plans for youth and useful safety plans for crisis situations. The YFC is responsible for making sure that the youth/families served have every opportunity to be successful in the home, school, and community settings. Also ensuring that the youth/families remain connected to people who are important to them. To support operational needs of this role, candidates must be based within a 50-mile commuting distance of Columbus, Georgia.

Key Responsibilities:

Work Environment:

This role is a full-time, 40-hour a week position that generally works in the field and requires occasional monthly in-office presence. To effectively support the youth, the expected work week is Monday through Friday with some evenings and weekend work as needed. Ability to drive long distances, mostly within the state of Georgia, is necessary.

QualificationsEssential

  • Adept at problem-solving and creativity in thinking & planning.
  • Respect for the value of youth voice, and skill in appropriately involving youth in their care plan.
Preferred:
  • Working knowledge and/or experience with MAAC.
  • Experience in coordination of resources and services with youth and families.
  • Experience working with and in a youth or family-centered practice model.
  • Previous experience in Wraparound.
