Job summary

Wayside Youth is seeking a compassionate Transitional Facilitator in Framingham to support 18–24 year olds in transitional housing. You will coordinate intake, provide case management, and help participants with life skills, employment readiness, and apprenticeship opportunities.

The role requires at least 1 year of relevant experience, a valid driver's license, and the ability to work in community settings. The position offers a salary of $21.00–$26.50 per hour plus a sign-on bonus.

Qualifications

• Bachelor's degree preferred; not required.
• Bi-lingual capacity extremely preferred.
• Minimum 1 year experience with adolescents or young adults, and with homeless populations.

Responsibilities

• Coordinate intake assessments for new participants.
• Schedule weekly case management meetings with participants.
• Develop, implement and evaluate service plans with participants.
• Provide life skills assessment and education for participants.
• Offer employment assistance, career development and apprenticeship matching.
• Build and maintain networks with community supports.
• Provide counseling, crisis intervention, and referrals.
• Monitor participants and ensure program documentation is completed.
• Attend agency meetings, trainings, supervision, and community meetings.
