Job description

Position Requirements and Responsibilities

Note: This position requires travel between York and Hanover hospitals.

Purpose:

To provide a high level of nutrition-related expertise within the interdisciplinary team management of all hospitalized patients.

Responsibilities:
  1. Ensure compliance with outpatient billing guidelines and perform necessary documentation to meet regulatory requirements.
  2. Provide in-service training or CEU opportunities to the multidisciplinary team at the department or facility level.
  3. Formulate a Nutrition Diagnosis and identify Malnutrition through Nutrition Focused Physical Assessment and established criteria.
  4. Execute order writing appropriately, following the Dietitian Delegation Order system protocol.
  5. Provide nutritional education and counseling tailored to patient/family learning needs and abilities.
  6. Communicate effectively as a collaborative team member within the interdisciplinary care team.
  7. Mentor and train dietetic students, interns, and support personnel, providing feedback for improvement.
  8. Implement timely interventions based on Nutrition Risk level and the Plan of Care.
  9. Deliver quality patient care to diverse populations using Medical Nutrition Therapy, including assessment, goal setting, and planning.
  10. Lead one departmental initiative annually, endorsing evidence-based practices.
  11. Serve as the nutrition liaison/expert to assigned units and participate in interdisciplinary committees to improve nutrition care processes.
  12. Demonstrate professional writing skills adaptable to all health literacy levels in preparing materials and presentations.
