Job summary

Qualifications

Responsibilities

• Provides personal care and escort assistance per individual treatment plans (peri-care, grooming, dressing, bathing, toileting, feeding, ambulation).
• Reports changes in participant condition to supervisor.
• Communicates health and safety concerns under supervision.
• Assists with social and recreation activities and daily physical activities.
• Implements rehabilitation and nursing staff-directed interventions; assists with transfers and mobility.
• Assists with meal service and monitors during mealtimes.
• Documents services rendered daily.
• Completes other projects and duties as assigned.

Position Information:

• Department: PACE- Clinic
• Salary Grade: 301 - $47,840 - $64,584 ($23.00 - $31.0500)
• Work Arrangement: Full Office
• Work Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• Expected Assignment Duration: Up to six (6) months and/or 960 hours

Duties & Responsibilities:

• 95% - Program Support
• Participates in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability.
• Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.
• Provides personal care and escort assistance in accordance with individual treatment plans including peri-care, grooming, dressing, showering, bathing, toileting, feeding and ambulation.
• Reports visible changes in the participant's condition to the Day Center Supervisor and PCA Lead.
• Communicates participant health and safety concerns under the guidance and supervision of the Day Center Supervisor and PCA Lead.
• Demonstrates safe practices utilizing proper body mechanics and compliance with universal precautions.
• Participates in social and recreation activities with participants as appropriate.
• Conducts daily physical activities, ergonomically, while assisting frail elders to avoid injury.
• Implements interventions to training, as directed by rehabilitation and nursing staff, including use of mechanical assistance for participant transfers, mobility and patient care.
• Assists with food service delivery and monitoring during mealtimes.
• Documents services rendered daily.
• 5% - Other
• Completes other projects and duties as assigned.

Minimum Qualifications:

• High school diploma or equivalent PLUS 1 year of experience with the frail or elderly population required; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
• 6 months of clinical experience as a Personal Care Aide, CNA, LVN or MA required.
• CPR and First-Aid Certification required or must be obtained within six (6) months of date of hire.
• Complete and pass the PACE approved curriculum for Personal Care Aides upon hire.
• Complete and pass the ServSafe Food Handler Certificate upon hire.

Preferred Qualifications:

• Bilingual in English and in one of CalOptima Health's defined threshold language (Arabic, Chinese, Farsi, Korean, Russian, Spanish, Vietnamese).

Required Licensure / Certifications:

• Completion of Home Health Aide (HHA) or Medical Assistant (MA) program, a Certified Nursing Assistant (CNA) or Licensed Vocational Nurse (LVN) license required.

Knowledge & Abilities:

• Develop rapport and establish and maintain effective working relationships with CalOptima Health's leadership and staff and external contacts at all levels and with diverse backgrounds.
• Work independently and exercise sound judgment.
• Communicate clearly and concisely, both orally and in writing.
