WV Intensive Clinical Care Coordination Specialist (Multiple Positions Available)

Position Details:

The WV Intensive Clinical Care Coordination Team (WVICCC) focuses on youth in out-of-state group residential and psychiatric facilities. The team aims to bring youth back to WV into community services that prevent higher levels of care. Responsibilities include making recommendations, developing, and implementing discharge plans, and working with providers and community partners. The Clinical Specialist will conduct assessments such as CANS and CAFAS.

Applicants can be located anywhere in WV or bordering states but must have experience working with WV youth. The position is remote.

Responsibilities include:

  • Developing discharge plans
  • Completing CANS and CAFAS assessments
  • Coordinating with providers and agencies for services
  • Follow-up with families post-discharge
  • Participating in treatment teams
  • Data collection and reporting

Location: OTHR - Other Off-Campus Location

Salary Range: $66,657

Qualifications:

  • Minimum M.A. or M.S. in Human Services (Counseling, Psychology, Social Work)
  • Must be licensed or working towards licensure in Psychology, Counseling, or Social Work
  • Experience working with children with intensive needs preferred
  • Ability to work remotely and flexible hours

Preferred: CANS and CAFAS certifications (not mandatory)
