The WV Intensive Clinical Care Coordination Team (WVICCC) will focus on youth who are out-of-state in group residential facilities and psychiatric residential treatment facilities. The goal of the team is to bring youth back to WV from out-of-state into community level services that are effective in keeping the youth at home and not returning to a higher level of care.

The team will make recommendations, assist with the development of the discharge plan and implementation of that plan. This team will work with the out-of-state providers, DHHR case workers and community partners to accomplish this. The Clinical Specialist will work with a team to identify all needs of the child and develop and implement discharge plans for the children described above. The Clinical Specialist will be responsible for completing the Child and Adolescent Needs Assessment (CANS) and the Child and Adolescent Functioning Assessment (CAFAS).

This position is a hybrid position. This person would also be responsible from providing Critical Incident Stress Management (CISM) for DoHS in Southern WV.

Applicants must be located in Southern WV to perform Crisis Incident Stress Management. The position is remote, except when needed to respond to critical incidents.

Duties And Responsibilities

  • Develop and implement the discharge plan for each child.
  • Complete the Child and Adolescent Needs Assessment (CANS) and the Child and Adolescent Functioning Assessment (CAFAS)
  • Work with Out-of-State Providers, DoHS, Aetna to secure services for children by discharge.
  • Follow-up with families after discharge to reduce recidivism
  • Participate in OOS treatment teams
  • Collect required data and reports.
  • Provide CISM individual or group services to Southern WV DoHS.

Must be able to work from home and work flexible hours.

Location OTHR - Other Off-Campus Location Salary Range 70,000

Required Qualifications

Education

Minimum M.A. or M.S. degree in Human Service related field (counseling, psychology, social work)

Experience

  • An understanding of youth with intensive needs.
  • Skills in using virtual platforms, such as ZOOM.
  • Strong Organizational Skills.

Preferred Qualifications

Education Experience

  • CANS and CAFAS certifications preferred but not mandatory.
  • CISM certification preferred but not required.
