Job summary

Partners for Kids and Families Inc in New Jersey is seeking a Care Manager to work with youth ages 5-21 and their families using wraparound supports.

You will coordinate services, develop ISP, maintain documentation, and participate in Child-Family Team meetings; Spanish-speaking encouraged.

Qualifications include a Bachelor's degree and two years of direct care experience; evenings and flexible schedule may be required; strong communication and cultural competence.

Qualifications

• Bachelor's degree in social work or related field.
• Two years experience providing direct care to children and families.
• Excellent communication, facilitation, assessment and system negotiation skills.
• Creativity and enthusiasm essential.
• Proficiency with Office 365 and videoconferencing platforms.
• Must have valid driver's license, auto insurance, and own vehicle.

Responsibilities

• Provide wraparound care management to youth and families using Child-Family Team meetings.
• Develop and monitor Individualized Service Plans (ISP) with CFT.
• Submit for authorization and coordinate services; maintain progress notes.
• Ensure confidentiality and HIPAA compliance.
• Respond to crises; participate in after-hours on-call rotation.
• Educate families and community about wraparound values.
• Connect families to community resources and supports.
