Job summary

Lutheran Social Services of SD seeks a dedicated Wraparound Care Coordinator to deliver strengths-based, family-driven case management using the CARES Wraparound model for children, youth, and families. You will coordinate services, connect resources, and help families develop and execute Plans of Care with ongoing involvement.

The role is full-time with travel requirements. LSS offers comprehensive benefits, a $600 sign-on bonus for external candidates, PTO frontload, holidays, a 401(k) match,

Qualifications

• Bachelor’s degree in social work or related field is required or can be substituted with relevant experience.
• Must possess or be eligible for licensure as a Social Worker or Social Work Associate in SD.
• Must be at least 21 years of age and pass background checks.

Responsibilities

• Provide information and intake services to families and partners regarding Wraparound services.
• Facilitate Family Team Meetings and guide families through the Wraparound process.
• Develop individualized Plans of Care and Crisis Plans with families.
• Assess family strengths, needs, and resources to support service planning.
• Maintain documentation in CareLogic and required program data reporting.
• Represent LSS professionally and participate in trainings.
• Travel to meet with families and coordinate services within the community.

HOURS:

Full-Time, 40 hours per week, occasional evenings as needed.

SALARY:

$21.00-$23.00 per hour, non-exempt. Dependent on experience and qualifications.

TRAVEL:

Travel is required. Mileage reimbursement.

As a Care Coordinator, you will implement the C.A.R.E.S. Wraparound model by providing strengths-based, family-driven case management and coordination of services for children, youth, and their families. The C.A.R.E.S. Wraparound model is a child abuse prevention and diversion program designed to support families early in the child welfare continuum and help them build upon their strengths, resources, and natural supports. You will work directly with families to bring the appropriate people and supports together, help families identify their strengths and needs, develop a plan, connect them with resources, and stay involved to help the plan succeed.

You will facilitate Family Team Conferencing and the Wraparound process, working collaboratively with families, youth, community partners, and referral sources to identify strengths and needs, develop individualized Plans of Care, and coordinate appropriate services and supports. You will promote a family-driven, youth-guided, community-based, culturally responsive, and trauma-informed approach.

RESPONSIBILITIES:

• Provide information and intake services to families, referral sources, and community partners regarding Wraparound services and the C.A.R.E.S. model.
• Facilitate Family Team weekly meetings and guide families through the Wraparound process, including developing individualized Plans of Care and Crisis Plans.
• Assess family strengths, needs, resources, and circumstances to support individualized service planning.
• Provide supportive counseling, resource coordination, and referrals to help children, youth, and families build on their strengths and access appropriate community and natural supports.
• Maintain respectful and professional relationships with families, referral sources, and community partners while providing ongoing consultation and coordination of services.
• Maintain timely and accurate documentation in required systems, including CareLogic, and complete required program data and reporting.
• Represent LSS and the C.A.R.E.S. Wraparound program professionally, participate in required meetings and training.
• Travel as required to meet with families, attend meetings, and coordinate services within the community.

QUALIFICATIONS:

Candidates must possess or be eligible to obtain licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners. Obtaining and maintaining licensure is a requirement of this position. Guidance through the licensure process will be provided by Lutheran Social Services of SD.

• Bachelor’s degree in social work or related field, plus one to two years related experience is required.
• Licensed Social Worker or Licensed Social Work Associate through the South Dakota Board of Social Work Examiners is preferred.
• Must be at least 21 years of age, is required.
• Must successfully complete all agency background checks, is required.
• Requires active auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.
