Position Summary
Phoenix Children’s is establishing a robust Wound Ostomy program that requires nurses to be certified within one year of employment. Nurses will work closely with patients, families, and providers to care for wounds and provide education for complex cases. We are hiring for two roles: one position 7 am–4:30 pm, the other 2 pm–10 pm, both 40 hours/week with varying schedules depending on need. Minimum requirements: one year of skin/wound/ostomy experience, minimum three years of clinical nursing experience (pediatrics preferred), and no new graduates. The role uses the nursing process and collaborates with the healthcare team to promote patient skin health through professional nursing care appropriate to patients’ age and condition, documented in care plans in accordance with evidence‑based practice and regulatory standards.
Position Duties
Assessment
- Complete nursing assessments of patient’s skin, wound, or ostomy condition using specialized knowledge.
- Identify risk factors for infants, children, and young adults with acute and chronic health conditions.
- Conduct initial and ongoing assessments in accordance with evidence based standards and policy.
Planning
- Collaborate with the health care team, patient and family to formulate individual family‑centered nursing plan of care for skin, wound, and ostomy care.
- Document plan of care and response on an ongoing basis as necessary.
- Participate in patient care rounds and conferences as appropriate.
- Formulate culturally sensitive nursing outcomes related to skin, wound, and/or ostomy care.
Implementation
- Determine and implement appropriate nursing plan of care or provide escalation as necessary.
- Provide expert hands‑on nursing care for patients with or at‑risk for skin breakdown, pressure injury, or with an ostomy.
- Contribute to team efforts by accomplishing required activities within established parameters to ensure compliance with specialty guidelines and documentation standards.
- Integrate specialized knowledge into patient and staff education, coaching, and mentoring as applicable.
Evaluation
- Monitor patients for treatment effectiveness and increased risk as condition changes; elevate concerns.
- Revise and update the nursing plan based on current evidence‑based practice and patient/family needs.
- Collect and analyze pertinent data.
- Continually evaluate patient responses to skin, wound, or ostomy care, provide teaching and discharge plans, and document progress.
- Assist in evaluating nursing processes, policies, procedures, protocols, and guidelines, making recommendations to improve outcomes related to ostomies, skin health, and pressure injury prevention.
- Participate and collaborate in skin health and pressure injury prevention initiatives.
Qualifications
Education
- Bachelor’s degree in Nursing from an ACEN or CCNE accredited graduate school of nursing.
Experience
- At least 3 years of clinical nursing experience, pediatrics preferred.
- At least 2 years of pediatric clinical experience.
- At least 1 year of skin/wound/ostomy experience.
- Experience with pressure injury prevention, wounds, and/or ostomies.
Special Skills
Additional Requirements
- Current state of Arizona RN license or equivalent compact state license, and ability to establish licensure by start date.
- Current BLS certification for Healthcare Provider from the American Heart Association.
- Current PALS certification for Health Care Providers from the American Heart Association.
- Wound & ostomy certifications required within one year of employment. Acceptable certifications include WCC, OMS, CWCN, CWOCN, CWON, CWCA, CWS, WCCOMS, CWCNOMS, CWCAOMS, WCSOMS.
