Job summary

Old Southwest Rehab and Nursing in Roanoke, VA is seeking a dedicated Wound Care Nurse (RN) to assess, treat, and monitor residents with wounds, promote healing, and prevent skin breakdown.

The ideal candidate holds an active Virginia RN license in good standing, with WCC/WOCN preferred; certification must be completed within the designated timeframe if not currently certified. Prior wound care or long-term care experience is preferred, along with strong assessment and communication skills.

Qualifications

• Virginia RN license in good standing.
• WCC, WOCN, or equivalent preferred; certification within designated timeframe if not current.
• Experience in wound care, long-term care, rehab, or skilled nursing preferred.
• Strong assessment, communication and organizational skills.
• Knowledge of skin integrity, pressure injury prevention and infection control.

Responsibilities

• Perform comprehensive wound assessments and develop individualized treatment plans.
• Provide wound care, dressing changes, and skin assessments.
• Monitor residents for pressure injuries and implement prevention strategies.
• Collaborate with physicians, rehabilitation staff and care team.
• Maintain accurate documentation and ensure regulatory compliance.
• Educate residents, families and staff on wound prevention and treatment.
• Participate in quality assurance and infection prevention initiatives.
