Job summary

Old Southwest Rehab and Nursing in Roanoke, Virginia is seeking a Wound Care Nurse (RN) to assess, treat, and monitor residents with wounds, ensuring the highest level of care and promoting healing. You will perform comprehensive wound assessments, provide dressing changes, monitor for pressure injuries, and collaborate with physicians and the interdisciplinary team to maintain skin integrity.

This role requires Virginia RN licensure in good standing; WCC/WOCN certification is preferred and must

Qualifications

• Virginia RN license in good standing.
• WCC/WOCN or equivalent preferred; must certify within designated timeframe if not certified.
• Experience in wound care, long-term care, rehab, or skilled nursing preferred.
• Knowledge of skin integrity, pressure injury prevention, and infection control.

Responsibilities

• Perform comprehensive wound assessments and develop individualized treatment plans.
• Provide wound care treatments, dressing changes, and skin assessments.
• Monitor residents for pressure injuries and implement prevention strategies.
• Collaborate with physicians, rehabilitation staff, and the interdisciplinary care team.
• Maintain accurate documentation and ensure regulatory compliance.
• Educate residents, families, and staff on wound prevention and treatment protocols.
• Participate in quality assurance and infection prevention initiatives.
