Job summary

Potomac Falls Health & Rehabilitation Center in Sterling, VA is seeking a dedicated Wound Care Nurse to assess, treat, and monitor residents with wounds and related conditions. You will collaborate with an interdisciplinary team to promote healing and ensure quality care in a long-term care setting.

The role requires active Virginia RN or LPN licensure, with WCC/WOCN certification preferred or obtainable. Prior wound care or long-term care experience is valued, along with strong documentation

Qualifications

• Current RN or LPN license in Virginia.
• Wound Care Certification (WCC/WOCN) preferred or willingness to obtain.
• Experience in long-term care, rehabilitation, skilled nursing, or wound care preferred.

Responsibilities

• Perform comprehensive wound and skin assessments on residents.
• Develop, implement, and evaluate wound care treatment plans.
• Monitor and document wound progress and treatment effectiveness.
• Coordinate care with physicians, nursing staff, and therapists.
• Educate residents, families, and staff on wound prevention and treatment.
• Ensure compliance with regulatory requirements and facility policies.
• Maintain accurate clinical documentation in the EMR.
• Participate in quality improvement and infection prevention initiatives.

The ideal candidate will be responsible for assessing, treating, and monitoring residents with wounds, skin conditions, and related healthcare needs while collaborating with the interdisciplinary team to promote optimal healing outcomes.

Responsibilities

• Perform comprehensive wound and skin assessments on residents.
• Develop, implement, and evaluate wound care treatment plans.

Qualifications

• Current RN or LPN license in the Commonwealth of Virginia.
• Wound Care Certification (WCC, WOCN, or equivalent) preferred, or willingness to obtain certification.
• Previous experience in long-term care, rehabilitation, skilled nursing, or wound care preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within a multidisciplinary team.
