Job summary

Cherrydale Post Acute is seeking a Wound Care Nurse (LPN) to deliver specialized skin and wound care for residents under physician and nursing leadership. You will promote healing, prevent complications, and contribute to improved resident outcomes.

Responsibilities include direct wound care, progress documentation, and collaborative care planning within a team-focused environment. Prior wound care experience and SC LPN licensure are strongly preferred.

Qualifications

• Active LPN license in South Carolina.
• Graduate of an accredited Practical Nursing program.
• Current CPR certification required.
• Experience with PointClickCare (PCC) preferred.
• Prior wound care experience strongly preferred.
• Strong clinical judgment and attention to detail.

Responsibilities

• Provide direct wound care and skin treatments per physician orders.
• Assess, monitor, and document wound progress and treatment effectiveness.
• Assist in developing and modifying individualized care plans.
• Collaborate with the interdisciplinary team for optimal outcomes.
• Educate staff on proper wound care techniques and prevention strategies.
• Maintain accurate and compliant clinical documentation.
• Ensure adherence to regulatory, safety, and infection control standards.

Position Summary

As a Wound Care Nurse (LPN) , you will be responsible for providing specialized skin and wound care to residents under the direction of the physician, Director of Nursing, and/or Medical Director. You will play a key role in promoting healing, preventing complications, and improving overall resident outcomes.
