Job summary

Jobtailor is seeking an experienced wound care nurse in South Carolina to assess and treat wounds, monitor healing, and educate residents and families. The role collaborates with physicians, the Medical Director, and the Interdisciplinary Care Plan Team to develop individualized treatment plans and ensure quality care.

You will provide direct nursing care, assist with care plan updates, and ensure compliance with HIPAA and regulatory standards while promoting resident dignity and comfort.

Qualifications

  • Previous wound care experience required.
  • Current, active RN or LPN license in good standing.
  • Minimum of 2 years of nursing experience.
  • LPN must hold a Wound Certification.
  • Nursing Degree from an accredited college or university.
  • Current CPR certification.
  • Knowledge and experience with PCC preferred.
  • Maintain required continuing education for licensing.
  • In good standing with State Board of Nursing.

Responsibilities

  • Assess, manage, and treat wounds, pressure injuries, skin tears, ulcers, and dermatologic conditions.
  • Complete comprehensive skin assessments and quarterly dermatologic evaluations.
  • Develop individualized treatment plans with the Interdisciplinary Care Plan Team.
  • Provide progress reports and document wound healing status.
  • Monitor wound healing, adjust care plans as needed, and report deterioration.
  • Educate residents and families on prevention and treatment plans.
  • Participate in staff education and wound care training.
  • Maintain HIPAA-compliant documentation and regulatory compliance.

Job description

  • Assess, manage, and treat wounds, pressure injuries, skin tears, ulcers, and other dermatologic conditions
  • Complete comprehensive skin assessments and quarterly dermatologic evaluations
  • Partner with physicians, the Medical Director, Director of Nursing, therapy, dietary, and the Interdisciplinary Care Plan Team to develop individualized treatment plans
  • Provide accurate written and verbal reports regarding resident progress and wound healing
  • Monitor wound healing, update care plans, and recommend treatment modifications
  • Ensure pressure injury prevention interventions, including repositioning, nutrition support, and skin protection
  • Provide direct nursing care during emergencies while promoting resident comfort and dignity
  • Educate residents and families on wound prevention and treatment plans
  • Participate in staff education and provide in-service wound care training
  • Monitor wound care supplies and equipment inventory
  • Maintain accurate documentation and compliance with regulations
  • Protect resident privacy and maintain HIPAA compliance
  • Report resident concerns, suspected abuse, safety issues, occupational exposures, and information security concerns
  • Foster positive working relationships with physicians, consultants, nursing staff, and families
  • Communicate with residents and families about plans of care and treatment protocols
  • Carry out physicians' orders, including medication and treatment
  • Observe and assess wounds and pressure sores, notifying physicians of deterioration or infection signs
  • Assist with preparing and updating care plans
  • Assist physicians and healthcare providers during visits and examinations
  • Notify families and attending physicians of significant treatment-related issues
  • Participate in orientation and ongoing nursing staff training
  • Report significant findings or changes to the RN Supervisor and/or Director of Nursing
  • Participate in facility surveys and inspections
  • Assist with overall supervision of nursing staff
Requirements
  • Previous wound care experience required
  • Current, active, unrestricted RN or LPN license in good standing
  • Minimum of 2 years of nursing experience
  • LPN must already hold a Wound Certification
  • Nursing Degree from an accredited college or university
  • Current CPR certification
  • Knowledge and experience with PCC preferred
  • Must maintain required continuing education for licensing
  • Must remain in good standing with the State Board of Nursing
  • Ability to read technical procedures and policy and procedure manuals
  • Ability to effectively present information and respond to managers and employees
  • Ability to apply fractions, percentages, ratios, and proportions to practical situations
  • Ability to solve practical problems and interpret written, oral, diagram, or schedule instructions
  • Ability to perform frequent standing/walking, reaching, pushing/pulling, talking/hearing, tasting/smelling, and lifting up to 50 pounds
  • Ability to perform occasional climbing, balancing, stooping, kneeling, crouching, or crawling
