Job summary

Legacy Health Services seeks an experienced Wound Care Nurse to join the Parkside Villa team in Middleburg Heights, Ohio. The role handles assessments, plan development, and wound management for residents with acute and chronic wounds in collaboration with an interdisciplinary team.

The position emphasizes regulatory compliance, education for staff, and quality improvement in a resident-centered care environment. Part-time and full-time options are available in a skilled nursing setting.

Qualifications

Current and active RN license (OH preferred) with LPN accepted.
Minimum 1–2 years of nursing experience, long-term care preferred.
Wound care certification (WCC/WOCN/CWCA) preferred or willingness to obtain.

Responsibilities

Conduct comprehensive wound assessments and monitor pressure injuries and wounds.
Develop individualized wound care plans based on best practices.
Perform wound treatments, debridement within scope, dressings, and skin care.
Document assessments and progress in the EMR accurately and timely.
Lead weekly/biweekly wound rounds with the care team.
Educate staff on wound prevention and skin integrity protocols.
Monitor skin integrity metrics and assist with quality initiatives.
Ensure CMS and facility policy compliance and survey readiness.
Provide clinical expertise during resident care conferences.
