Job summary

Hartford HealthCare at Home is seeking a Wound Care RN to participate in the daily management and delivery of wound care within the Wound program. You will ensure outcome-based, cost-effective care and collaborate with Wound team members to achieve exceptional patient outcomes.

The Wound Program RN will direct and coordinate all client care services for wound care and medical/surgical needs, determine staffing and scheduling, and participate in training and performance improvement activities.

Qualifications

• Associates degree or diploma in nursing; Wound/Ostomy certification; BSN preferred.
• Minimum two years nursing experience in clinical specialty area; home care experience preferred.
• RN license required; Wound/Ostomy certification within 6 months of hire.
• Demonstrated communication, leadership, and problem-solving abilities.
• MS Office proficiency (Outlook, Word, Excel, PowerPoint) and MS Project.

Responsibilities

• Deliver wound care and oversee clinical staff to maintain care standards.
• Coordinate staffing and scheduling to meet service demands.
• Collaborate with WOCN staff to plan interventions for complex cases.
• Develop and implement program goals and improve patient outcomes.
• Participate in training, coaching, and onboarding for the wound program.
