Job summary

Hartford HealthCare at Home is seeking an experienced Wound Care RN to join our CT-based wound program. You will lead clinical staff, coordinate wound care delivery, and ensure outcome-based treatment plans in patients’ homes.

You will collaborate with WOCN staff and interdisciplinary teams to design interventions, manage caseloads, and monitor quality metrics while advancing the program’s goals and maintaining high standards of care.

Qualifications

• RN license required; Wound/Ostomy certification within 6 months of hire
• Associates degree or diploma in nursing; Bachelor's degree preferred
• Minimum of two years in a clinical nursing specialty; home health experience preferred
• Strong communication, leadership and problem‑solving skills
• Proficiency with MS Office suite (Outlook, Word, Excel, PowerPoint, Project)
