Job summary

ADP, Inc. is seeking a Wound Care Registered Nurse (RN) in Colorado Springs, CO to oversee wound management and skin integrity across our community, combining hands-on clinical care with staff education, leadership, and quality improvement to support resident-centered care.

The RN will perform assessments, develop treatment plans, monitor infections and pressure injury risks, mentor staff, and ensure accurate EMR documentation while complying with CMS and Colorado regulations. CPR is required.

Qualifications

• Current CPR certification.
• Minimum 1 year of nursing experience in Skilled Nursing or related healthcare setting.
• Wound care certification preferred; ability to obtain certification within 90 days of hire.
• Experience with PointClickCare or other EHR systems is preferred.
• Knowledge of CMS and long-term care regulations preferred.

Responsibilities

• Complete comprehensive skin and wound assessments, including measurements, staging, photographs, and documentation.
• Develop and monitor individualized wound care treatment plans and perform treatments within scope of practice.
• Identify residents at risk for pressure injuries and lead prevention initiatives.
• Monitor wounds for infection, changes in condition, and treatment effectiveness.
• Participate in wound rounds and collaborate with nursing, physicians, therapy, dietary, and other interdisciplinary team members.
• Educate and mentor nursing staff on wound care, skin integrity, prevention, treatment, and documentation.
• Provide wound care education to residents and families.
• Maintain accurate documentation in the electronic medical record.
• Monitor wound care quality indicators and participate in QAPI initiatives.
• Support compliance with CMS, Colorado regulations, infection prevention standards, and community policies.
• Assist with surveys, audits, and regulatory inspections.
• Participate in rotating nurse on-call responsibilities.
