Job summary

Lincolnshire Health and Rehabilitation Center in Merrillville, Indiana seeks a dedicated Wound Nurse to join our skilled nursing team. The role focuses on developing individualized wound care plans, conducting treatments, and promoting evidence-based practices.

You will collaborate with interdisciplinary teams, educate patients and families, and participate in quality improvement initiatives. Indiana RN licensure and WOC certification or willingness to obtain are required.

Qualifications

• 1 year of experience as a registered nurse; wound care experience preferred.
• Current licensure as a registered nurse in Indiana (state license).
• Certification in wound, ostomy, and continence (WOC) nursing or willingness to obtain within a specified timeframe.

Responsibilities

• Develop and implement individualized wound care plans for patients, utilizing evidence-based practices and protocols
• Conduct wound assessments and treatments, including debridement, dressing changes, and wound cleansing
• Collaborate with interdisciplinary teams to ensure comprehensive care and coordination of services
• Provide education and training to patients, families, and caregivers on wound care techniques and self-management strategies
• Participate in quality improvement initiatives to enhance patient outcomes and reduce readmissions
• Maintain accurate and detailed records of patient care and wound assessments
