Job summary

Signature HealthCARE of South Pittsburg Rehab & Wellness Center seeks experienced RN candidates to deliver quality nursing care within a 165-bed facility. You will administer medications, assist with ADLs, and collaborate with an interdisciplinary team to ensure resident comfort, dignity, and safety.

We require an active RN license in Tennessee and strong clinical skills, with familiarity in EHR systems and documentation.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with ADLs and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during shifts per policy.
• Monitor and observe residents’ status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
