Job Title: Wound Care RN
Location: Brooklyn, NY
Facility: Spring Creek Rehabilitation & Nursing Care Center
Shift: Full-Time, 9:00 AM – 5:00 PM
Salary: $115,000 per year

Job Description:
Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and dedicated Wound Care RN to join our team in Brooklyn, NY. In this full-time role, you will provide high-quality wound care to our residents, collaborating with a multidisciplinary team to ensure optimal healing and comfort.

Key Responsibilities:

  • Assess residents/patients with existing or newly developed wounds.

  • Collaborate with physicians to establish and implement appropriate treatment plans.

  • Lead weekly wound rounds with the care team; monitor and document wound progress.

  • Update and maintain a weekly tracking form for each wound.

  • Communicate promptly with physicians if a wound is not responding to treatment.

  • Maintain and distribute weekly decubitus statistics to the wound care team every Monday.

  • Conduct in-service training for staff on proper wound care procedures.

  • Perform quarterly Quality Assurance reports to ensure compliance with each resident's wound care plan.

  • Observe and evaluate dressing changes to ensure staff competency.

  • Provide wound care education to residents and their families as needed.

  • Attend continuing education sessions to remain current with best practices and standards of care.

Requirements:

  • Active New York State Registered Nurse (RN) license

  • Wound Care Certification

  • Infection Control Certification

  • Minimum of 1 year of experience; nursing home experience preferred
