Job summary

Jessica Wong Nurse Practitioner in California seeks a Registered Nurse to join the team and manage patient care plans with a detail‑driven, compassionate approach. You are an RN who delivers personalized care and communicates effectively for the patient\'s best interest, with willingness to travel.

This on‑the‑road position involves home or facility visits across Los Angeles County, Orange County, Inland Empire and surrounding areas, coordinating with Providers and maintaining accurate EHR

Qualifications

• Must be a currently licensed RN in the state of employment.
• Experience as an RN is preferred.
• First aid/CPR certification is required.
• Wound Care certificate is preferred.

Responsibilities

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Administer wound care and monitor patients for improvement or decline
• Document any changes in patient conditions and report on changes
• Maintain accurate records of patient care and discuss observations with the Providers
• Utilize electronic health record (EHR) systems like WoundAlly for accurate documentation of patient care activities and medical records.
• Visiting patients home, facilities, etc. in the Los Angeles County, Orange County, Inland Empire and surrounding areas.

Job Summary

We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart.
