Job Summary :
We are seeking a dedicated and compassionate Wound Care Nurse, Telehealth Coordinator within the skilled nursing environment.
The Wound Care Nurse, Telehealth Coordinator acts as the link between healthcare providers and patients, providing dressing changes to wounds under direct supervision, utilizing real-time imaging technologies.

Responsibilities :
- Assist in conducting weekly virtual wound rounds using telehealth technology under the guidance of wound care specialists, nurses, or healthcare providers.
- Photograph wounds using designated telehealth technology and ensure accurate documentation of images for clinical review.
- Help patients navigate telehealth platforms, troubleshoot technical issues, and ensure seamless virtual connections for appointments.
- Facilitate patient telehealth scheduling and provide education on virtual visits to patients and staff as needed.
- Administer wound care procedures, including dressing changes and rounds, following evidence-based practices and facility policies.
- Maintain confidentiality and adhere to HIPAA regulations during all telehealth interactions and documentation.
- Submit orders for wound care products as needed.
Qualifications :
- Graduate of an accredited nursing school with current CPR certification. Minimum one year of wound care experience required; wound care certification preferred. Must hold an active, unencumbered RN or LPN license in the state of practice.
- Excellent communication skills to convey medical information clearly to physicians, staff, patients, and families.
- Empathy, patience, and a genuine desire to provide quality healthcare both in-person and via telehealth.
- Commitment to patient confidentiality, privacy, and data security in accordance with HIPAA regulations.
- Ability to multitask and adapt to changing telehealth workflows and patient needs.
- Prior experience in SNF, LTC, or Assisted Living is preferred.
- Must have a valid driver’s license, own a personal vehicle for travel to facilities, and understand mileage reimbursement policies.
- This role starts as Part-Time with the potential to become Full-Time.
