Job summary

Sweetwater Care in Great Falls, Montana seeks a qualified nurse (RN or LPN) to provide primary skin and wound care for residents per physician orders. You will assess wounds, document findings, and coordinate with the care plan team to ensure comprehensive resident care.

The role requires two years of LPN/RN experience, state licensure, and ongoing wound care education; certification is preferred but not mandatory. This is a full-time position focused on high-quality patient outcomes.

Qualifications

  • Nursing degree from an accredited college or university or be a graduate of an approved LPN/RN program.
  • Two years of experience as an LPN/RN preferred (experience may vary by facility).
  • Continuing education in wound care and pressure injury staging required; certification preferred but not required.

Responsibilities

  • Identifies, manages, and treats skin conditions such as pressure injuries and ulcers.
  • Provide wound care on assigned residents according to physician orders and product guidelines.
  • Complete wound assessments and follow-up assessments; document all findings in the medical record.
  • Collaborate with the care plan team to develop comprehensive care plans.
  • Document care problems and changes in resident condition and report to appropriate staff.
  • Monitor preventive measures such as turning, pressure relief, nutrition, and toileting/incontinence care.
