Personic Health is seeking an RN/LPN for in-home wound care, traveling to patients’ homes and facilities. Training will be provided and paid for by Personic Health. You’ll join a physician-led team delivering care in patients’ homes with a focus on high-quality outcomes.

This mobile role requires reliable transportation and a commitment to patient safety, documentation, and ongoing professional development within a supportive work environment.

Qualificações

• RN or LPN licensure in good standing.
• Ability to travel to patients’ homes and facilities.
• Wound care knowledge and sterile technique.
• Strong communication and documentation skills.

Responsabilidades

• Provide in-home wound care to patients per clinician orders.
• Coordinate with physicians, caregivers, and facilities to plan care.
• Document assessments and treatment plans accurately.
• Maintain infection control and patient safety standards.

Wound Care Nurse – In Home Wound Care

Personic Health is seeking an RN/LPN to join our organization as a Wound Care Nurse. This is a mobile position, requiring individuals to have their own transportation to travel to patients’ homes and assisted living facilities. Training will be provided and paid for by Personic Health, with course fees covered.

40 hours per weekday Monday to Friday

On the road, including Home Care, Long-term care facilities, Nursing homes, & Outpatient settings
