Job summary

Gatesrehab seeks a Wound / Treatment Nurse (RN or LPN) to coordinate and provide wound care and prescribed treatments for residents. Qualifications

• Current, active RN or LPN license in the state.
• Experience in wound care and treatment nursing preferred.
• Strong knowledge of skin integrity and wound protocols.
• Excellent documentation and organizational skills.
• Ability to work independently and meet regulatory timelines.
• Strong communication and interdisciplinary collaboration.

Responsibilities

• Coordinate and administer physician-ordered wound care and treatments.
• Perform skin assessments on all new admissions and ongoing residents.
• Monitor, document, and communicate wound progress and interventions.
• Maintain accurate and timely wound documentation and treatment records.
• Participate in care planning, MDS support, and interdisciplinary collaboration related to wound care.
• Assist with tube feedings and related treatments as assigned.
• Order wound care supplies and maintain inventory levels.
• Participate in quality assurance, survey readiness, and audits related to skin integrity.
• Provide education and guidance to nursing staff on wound care practices.
• Complete incident reports and documentation related to skin or treatment concerns.
• Ensure treatment areas are clean, safe, and infection-control compliant.
