Qualifications

• Current, active LPN or RN license in Tennessee.
• Wound care certification required or obtainable within 90 days.
• Strong clinical assessment and communication skills.
• Ability to collaborate with an interdisciplinary health care team.

Responsibilities

• Collaborate with the interdisciplinary care team to ensure accurate patient assessments and coordinated care.
• Develop and implement individualized patient care plans.
• Assess, monitor, and document wounds and healing progress.
• Provide and oversee wound care treatments per standards and physician orders.
• Maintain open communication with patients, families, and health care team members.
• Assist with medication administration and treatments as needed.
• Ensure accurate and timely clinical documentation.
• Perform additional nursing and patient care duties as assigned.
