Job summary

Waverly Rehabilitation And Healthcare Center in Virginia is seeking a full-time Wound Care Nurse to develop and oversee individualized wound care plans in collaboration with an interdisciplinary team.

You will lead skin preservation strategies, ensure protocol compliance, participate in weekly rounds for high-risk patients, provide hands-on wound care, organize restorative programs, and meet regulatory requirements.

Qualifications

• Active RN or LPN license in Virginia.
• Experience in wound care preferred.
• Experience in long-term care settings preferred.

Responsibilities

• Develop and implement individualized wound care plans with an interdisciplinary team.
• Preserve skin integrity by applying skin preservation strategies.
• Ensure wound care protocols are approved by medical staff.
• Participate in weekly multidisciplinary wound and skin rounds for high-risk patients and severe wounds.
• Provide hands-on wound care for difficult and non-healing wounds.
• Organize and implement the restorative program.
• Ensure all regulatory requirements are met.

Summary/Objective

In this role, you will develop and ensure the wound care plan specific for each patient in collaboration with an interdisciplinary team.

• Preserving the integrity of the skin by implementing and monitoring skin preservation strategies.
• Ensuring all wound and skin care protocols are approved by the medical staff.
• Participating multidisciplinary wound and skin rounds on a weekly basis for high risk patients and patients with current pressure ulcers and other significant wounds.
• Providing hands-on wound care for all difficult and non-healing wounds.
• Organize and implement the restorative program.
• Ensuring all regulatory requirements are met.

Qualifications

• Current State License as an RN/LPN
• LTC/SNF experience preferred
