Job description

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our wound care team in Troy, NY. This full-time position offers a Monday through Friday schedule, with no weekends or call responsibilities. The NP will provide both inpatient and outpatient care, working closely with colorectal and urology surgeons. The role involves pre-surgery site markings, patient education for ostomy care, and ongoing post-procedure follow-up.

As a Wound Care Nurse Practitioner, you will be responsible for conducting comprehensive assessments of ostomy patients, evaluating wound status, skin integrity, and overall health. You will collaborate closely with colorectal and urology surgeons for pre-surgery site markings and help ensure that patients and their families receive thorough education on ostomy care. This includes providing information on what to expect from the procedure, life adaptations, and post-procedure follow-up care. You will develop and implement individualized care plans focused on optimizing wound healing and preventing complications. Additionally, you will educate patients on proper ostomy care techniques, skin maintenance, and ways to prevent complications. Throughout this process, you will work collaboratively with physicians, nurses, and other healthcare professionals to coordinate care, ensuring continuity of treatment and the best outcomes for patients.

Competitive salary range with incentives: $114,430-123,584.

Qualifications:

  • At least one year of NP employment experience is preferred, particularly in wound care management and ostomy care. However, we will also consider upcoming 2025 NP graduates.
  • Board Certified Nurse Practitioner.
  • Active New York State NP license or ability to obtain one before start date.
  • DEA, Wound Care Certification (or willingness to obtain).
