• SWOC or wound care certification preferred.

Responsibilities

• Provide primary skin and wound care under supervision, focusing on resident comfort and healing.
• Assess, document, and treat skin conditions including wounds, ulcers, and infections.
• Collaborate with interdisciplinary care team to develop individualized care plans.
• Maintain accurate records and treatment documentation; participate in ongoing education.
• Assist with resident admissions, discharges, and transitions of care.
