Job summary

Elevate Health and Rehabilitation, part of the Ascent Healthcare Management family, seeks a Wound Nurse to provide nursing care to residents per physician and in accordance with standards of nursing practice and regulations. You will perform preventive skin care, wound care, monitor nutrition, document thoroughly, and coordinate with physicians and families.

Qualifications

• Active state nursing license (RN or LPN)
• 1–3 years of related nursing experience
• Current CPR certification
• Excellent technical, assessment, and documentation skills
• Customer service oriented with ability to work under pressure

Responsibilities

• Perform preventive skin care, dressing changes and wound care.
• Monitor turning and positioning of residents.
• Monitor treatment of decubitus ulcers with documentation and communication to physician/family.
• Make rounds with physician as needed for residents with decubitus or catheter care.
• Assess weekly nutrition/hydration of residents with ulcers and adjust intake as required.
• Maintain clean, orderly treatment cart and restock supplies.
• Follow Infection Control policies and procedures.
• Other duties as assigned.
