Job summary

Regency Hermiston seeks a certified wound care/treatment specialist to join the center clinical team, working under physician orders to develop and implement personalized wound care plans that follow best practices.

You will perform wound/skin assessments, create evidence-based care plans, and communicate changes to the healthcare team. You will collaborate with staff in- room for training, review patient charts, deliver skilled nursing care, and rotate nursing duties as needed.

Qualifications

• Must possess a nursing license in the state of employment.
• Active wound care certification required.
• Maintain knowledge of federal, state, and local regulations and best practices.

Responsibilities

• Perform accurate wound/skin care assessments for each resident.
• Develop and implement evidence-based wound care plans.
• Communicate patient status changes to the healthcare team and physician.
• Visit in-room with staff for collaboration, education, and competency checks.
• Review charts of wound care patients and deliver skilled nursing care.
• Manage nursing duties on a rotating basis.

You will:

• Perform accurate and comprehensive wound/skin care assessments of each assigned resident.
• Develop and implement an evidence-based care plan for each wound care patient.
• Communicate patient clinical status changes to the healthcare team and physician.
• Visit in-room with staff for collaboration, education/training, evaluating competency, and performing skills checks.
• Review charts of wound care patients as assigned.
• Deliver skilled nursing care.
• Manage nursing duties on a rotating basis.

You currently:

• Possess a nursing license in the state of employment.
• Possess active wound care certification.
• Maintain current knowledge of federal, state, and local regulations and inform on appropriate issues.
• Maintain current wound care/treatment practices.
