Job summary

CareMed seeks a Wound Care Nurse Practitioner to serve residents in long-term care facilities. You’ll diagnose and treat wounds, order tests, and collaborate with an interdisciplinary team to optimize patient outcomes. Ohio licensure and board certification are required; DEA is optional.

Experience with wound care products and wound VACs is preferred. The role emphasizes proactive care, documentation in EMR, and communicating changes in patient condition to nursing home staff and primary care

Qualifications

  • Active, unrestricted Ohio NP license.
  • Board certification in nurse practitioner.
  • Experience with wound care in long-term care settings.
  • Experience diagnosing and treating diabetic, arterial, venous wounds.
  • Familiar with wound care products (collagens, alginates, foams, silvers).
  • Experience with wound VACs and DME devices.
  • Wound certification is preferred or willingness to certify.
  • Experience working in a long-term care setting.
  • Ability to use an EMR.
  • Ability to work in an interdisciplinary care team.

Responsibilities

  • Provide wound care services for long-term care patients in skilled nursing facilities.
  • Prescribe medications, including controlled substances per OH law.
  • Order, review, and follow up on treatments and diagnostic testing.
  • Communicate with nursing home staff on changes in condition.
  • Collaborate with the primary care physician.
