Job summary

JMC Jefferson Medical Center is seeking an experienced Wound Care LPN to provide direct nursing care in an outpatient setting. You will screen patients, support physicians, and administer medications while delivering high-quality wound care.

Must hold an active LPN license and obtain BLS within 30 days of hire. The role involves collaboration with nurses and physicians to manage wound care and ostomy needs in both inpatient and outpatient contexts.

Qualifications

• Current LPN license issued by the state or multi-state LPN license via eNLC.
• Basic Life Support certification obtained within 30 days of hire date.

Responsibilities

• Provide direct nursing care to patients in an outpatient medical office setting.
• Obtain and record vital signs and health data; screen patients for chief complaint.
• Administer intramuscular, subcutaneous, and oral medications as ordered.
• Assist Provider and RN during examinations and procedures.
• Collaborate with healthcare team to plan and deliver patient care.
• Provide wound care management including assessment and dressing.

Ensure patient's chart is available. Escort patients to and from exam room. Serves as a resource and educator for other healthcare professionals, patients, families and community. Collaborates with nurses, physicians and other healthcare providers to plan and deliver patient care. **Patient care setting is BOTH inpatient and outpatient.**1. Ensures patients and caregivers receive education about their condition, therapies, and dressings.2. Provides comprehensive wound management including assessing causative factors, nutritional needs and provide topical therapies to inpatient patients.3. Provides instruction on proper wound dressing.4. Assesses and treats draining wounds and fistulae.5. Reviews and modifies pouch system problems.6. Cross trains and supports outpatient wound care center as needed.7. Provides recommendations on strategies to help improve and grow the inpatient and outpatient wound care program.8. Collaborates with physician and/or care management as needed to plan and deliver patient care.9.
