Job summary

Emerald Care Center Claremore is seeking a compassionate Wound Care/Infection Preventionist Nurse to join our Nursing team. This dual-role position focuses on wound care assessment, planning, treatment, and infection surveillance for residents.

RN or LPN with active state license required; WCC and IP certifications are required or must be obtained within 30 days. Minimum 2 years of wound care experience preferred in skilled nursing or similar settings.

Qualifications

• Must hold RN or LPN license with a valid state nursing license.
• Wound Care Certification (WCC) and Infection Preventionist (IP) certification required; willing to obtain within 30 days if not certified.
• Minimum 2 years of wound care experience in a skilled nursing or similar healthcare setting.

Responsibilities

• Assess and plan individualized wound care and infection prevention for residents.
• Administer wound care treatments including dressing changes and debridement with proper infection control.
• Lead infection prevention surveillance, document findings, and report to QA committee.
• Collaborate with physicians, nurses, and staff to coordinate care and educate on protocols.
• Monitor progress, maintain accurate records, and ensure regulatory compliance.
• Educate residents and families on wound care and prevention strategies.
• Participate in quality improvement initiatives related to wound care and infection prevention.
• Respond to urgent wound care needs and manage complex cases with the team.
