Twin Cities Physicians in Willmar, MN is seeking a Physician Assistant (PA-C) to serve as a Wound Care & Geriatric Rounding Provider in senior living communities.

This mobile role combines advanced wound management with geriatrics and chronic disease management, delivering care across facilities and reducing hospitalizations.

Qualifications

• Active Minnesota PA-C license in good standing.
• Experience in wound care, geriatrics, family medicine, internal medicine, primary care, or post-acute settings preferred.
• Strong clinical judgment and confidence managing medically complex patients.
• Valid driver's license and reliable transportation.
• Passion for delivering outcomes-driven, patient-centered senior care.

Responsibilities

• Provide on-site rounding, primary care, and advanced wound care services.
• Manage pressure injuries, diabetic ulcers, surgical wounds, skin integrity concerns, and complex chronic conditions.
• Perform assessments, diagnostics, treatment planning, and follow-up care.
• Drive chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
• Help reduce avoidable hospitalizations through proactive clinical intervention and continuity of care.
• Complete documentation, medication management, and lab coordination.
