Job summary

Twin Cities Physicians is seeking a Physician Assistant (PA-C) to join our wound care and senior living rounding team in the Greater Twin Cities Metro Area. This full-time role emphasizes on-site care across multiple senior living communities, focusing on preventing complications and improving quality of life for residents.

The PA will manage wound care, chronic disease oversight, and preventative care with interdisciplinary teams.

Qualifications

• Active Minnesota PA license in good standing.
• Experience in wound care, geriatrics, primary care, internal medicine, family medicine, or post-acute care preferred.
• Strong clinical judgment and communication skills.
• Valid driver's license and reliable transportation.
• Commitment to high-quality, patient-centered senior care.

Responsibilities

• Provide on-site rounding care throughout assisted living, memory care, and long-term care communities.
• Manage pressure injuries, diabetic ulcers, post-surgical wounds, and skin integrity concerns.
• Support chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, therapists, caregivers, and interdisciplinary teams.
• Communicate effectively with families, caregivers, and facility staff.
• Complete documentation, medication management, diagnostics, and lab coordination.
