Job summary

The Pearl of Downers Grove is seeking a Wound Care Director to lead a skilled nursing team in Downers Grove, IL. This pivotal role focuses on delivering high-quality wound care and improving patient outcomes.

You will collaborate with physicians, mentor staff, and drive wound care programs, rounds, and education. The facility offers competitive benefits and a supportive, outcome-focused environment.

Qualifications

• Currently licensed as RN (Required).
• Wound Care Certified (Preferred).
• Minimum 3 years’ experience in working within a skilled nursing, short term rehabilitative environment.
• Prior experience with Wound Rounds (Preferred).

Responsibilities

• Evaluate wounds and collaborate with physician to ensure optimum delivery of care for wound healing.
• Record, measure and document resident's wounds/wound progress.
• Initiate nursing care procedures in managing acute, chronic and/or traumatic wounds.
• Track and monitor (for infection and healing) trending and record skin conditions and wounds/wound management.
• Coordinate treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation, and outcomes for patients throughout the facility.
• Provide patient, family and staff education regarding wound management and therapies.
• Act as the resource person for physicians and clinical staff for wound management.
• Perform general nursing duties as needed.
• Participate in quality improvement plan for the facility.
• Develops skin care programs to prevent skin breakdown.
• Demonstrate knowledge and skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care, and incontinence.
• Demonstrate revision to plan of care as indicated by the patient’s response to treatment and evaluate overall plan for effectiveness.
