Job summary

The Pearl of Downers Grove is seeking a Wellness Nurse to monitor resident health, implement wellness plans, and coordinate with care teams to promote a safe and healthy environment.

As a key member of Pearl Healthcare, you will oversee nursing care in an assisted living setting, ensuring high-quality geriatric wellness and collaboration with physicians, nurses, and staff.

Qualifications

• Active LPN or RN license in Illinois (Required).
• Experience in assisted living, long-term care, or senior care (Required).
• Knowledge of geriatric care and wellness principles.
• Strong communication, organizational, and teamwork skills.

Responsibilities

• Assess and monitor resident health, including vital signs and chronic conditions.
• Administer medications and treatments within scope of practice.
• Collaborate with physicians, nurses, and care teams on wellness plans.
• Educate residents and staff on health, nutrition, and preventive care.
• Maintain accurate health records and document interventions.
• Support resident engagement in wellness programs.
