ALIYA Healthcare Consulting is seeking an experienced Wound Care Director to oversee assessment, prevention, treatment, and management of wounds in our skilled nursing facilities. The role partners with physicians, nurses, therapists, and dietitians to ensure evidence-based care and positive resident outcomes.
The ideal candidate holds an Illinois LPN-RN license, a nursing degree, and wound care certification.

We are seeking an experienced and compassionate Wound Care Director to oversee the assessment, prevention, treatment, and ongoing management of wounds across our skilled nursing facilities. The Wound Care Director will work closely with nursing leadership, physicians, nurse practitioners, therapists, dietitians, and facility staff to ensure high-quality, evidence-based wound care and positive resident outcomes.
Oversee and coordinate wound care programs across assigned facilities
Conduct comprehensive wound assessments and develop individualized treatment plans
Monitor wound healing progress and revise interventions as clinically appropriate
Collaborate with physicians, advanced practice providers, nursing staff, dietitians, and other members of the interdisciplinary team
Ensure accurate and timely wound documentation in accordance with facility and regulatory requirements
Monitor wound-related clinical outcomes and identify opportunities for improvement
Current LPN-RN license in the State of Illinois
Bachelor's degree in Nursing preferred
Minimum 1 year of clinical nursing experience, preferably in skilled nursing, long-term care, or post-acute care
Previous wound care experience required
Experience overseeing or coordinating wound care programs strongly preferred
Wound care certification such as WCC, CWCN, CWS, or CWOCN preferred
Strong knowledge of pressure injuries, diabetic wounds, venous and arterial ulcers, skin integrity, and wound prevention
