Solaris HealthCare Osceola is seeking a Wound Care/ Desk Nurse RN to join our team. The role focuses on comprehensive wound assessments, documentation, and coordinated care with patients, families, and medical staff within a skilled nursing setting.

You will develop individualized care plans, perform dressings and debridement as needed, educate patients, and participate in quality improvement initiatives while maintaining up-to-date records and staying current with best practices in wound care.

Qualifications

• Current and unrestricted RN license in Florida.
• Wound Care Certification strongly preferred.
• 2+ years of wound care experience preferred.

Responsabilités

• Conduct comprehensive wound assessments including wound measurements and documentation.
• Record Management.
• Communicating between patients, families, doctors, and other medical staff to keep treatments running smoothly.
• Scheduling appointments, prioritizing daily tasks for the unit, and organizing facility workflows.
• Develop and implement individualized care plans for patients with wounds.
• Perform wound care treatments, including dressing changes, debridement and other necessary interventions.
• Educate patients and their families on wound care, preventions strategies, and overall health maintenance.
• Collaborate with multidisciplinary teams to coordinate patient care and ensure optimal outcomes.
• Monitor and evaluate the effectiveness of wound care treatments and adjust plans as necessary.
• Maintain accurate and up-to-date patient records and documentation.
• Stay current with best practices, evidence-based protocols and new technologies in wound care.
• Participate in continuous quality improvement initiatives and contribute to the development of wound care policies and procedures.
• Provide guidance and support to nursing staff and other healthcare professionals on wound care management.

• Work may involve exposure to infectious diseases, hazardous materials and other conditions found in healthcare setting.
• Must be able to lift and move patients as needed.
• May require standing, walking & moving for extended periods.
