Job summary

Extended Care LLC is seeking a dedicated Wound Care Coordinator to oversee wound care management and skin integrity programs for residents. The RN or LPN will provide high-quality wound care, ensure preventative measures, and maintain compliance with federal and state regulations and facility policies.

Qualifications

• Active nursing license in the state.
• Current CPR certification.
• Minimum two years RN or LPN experience.
• Ability to lift up to 75 lbs and assist with patient mobility.
• Excellent verbal and written English communication.

Responsibilities

• Perform wound and skin care assessments using the nursing process.
• Conduct regular wound care rounds with timely documentation.
• Assess residents' skin integrity and document medical history related to wound care.
• Implement preventative wound care strategies in care plans.
• Coordinate with physicians and interdisciplinary teams for wound treatment needs.
• Initiate nutritional interventions to support wound healing.
• Provide direct resident care in accordance with facility policies.
• Maintain infection control and a safe, clean work environment.

Wound Care Coordinator Requirements:

• Active Nursing License (LPN/RN) as required by the state
• Current CPR certification
• Minimum two (2) years of experience as an RN or LPN
• Ability to lift and move patients and items up to 75 lbs
• Strong vision abilities including close, distance, peripheral, and depth perception
• Excellent verbal and written communication skills in English
• Ability to cope with the mental and emotional stress of the position
• Strong attention to detail and ability to prioritize wound care needs

As a Skilled, Compassionate Wound Care Coordinator, You Will:

• Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care
• Conduct regular wound care rounds, ensuring timely documentation and tracking of resident skin conditions
• Assess resident skin integrity and document medical history related to wound care
• Implement preventative wound care strategies in resident care plans
• Prioritize and execute nursing interventions for wound management
• Communicate with physicians and interdisciplinary teams regarding wound treatment needs
• Initiate nutritional interventions to support wound healing
• Perform direct resident care in compliance with facility policies and procedures
• Ensure residents with incontinence receive appropriate pericare and preventative treatments
• Maintain a safe and clean work environment, adhering to infection control protocols
