Job summary

Chrysalis Host Home in Henderson, NV seeks caring families to open their home to an adult with intellectual disabilities. As a Host Home Provider, you would live with and support a person, helping with daily living, scheduling medical appointments, and facilitating activities in a nurturing home environment.

This is a contracted position with compensation based on care level, including a room and board payment and a daily rate for days in care.

Qualifications

• All adults in the home must pass a Criminal Background Check
• Must be 21 years of age or older
• Must have experience providing support to individuals with disabilities
• Meet home/apartment safety and health requirements
• Complete pre-service training requirements
• Capable of communicating well in both verbal and written form
• Must have and maintain a valid driver's license and clean driving record
• Have good judgment
• Have a positive attitude

Responsibilities

• Transporting, scheduling, and attending medical appointments with the individual.
• Transporting to and from activities as needed.
• Completing monthly paperwork.
• Maintaining a safe and clean environment for the individual.
• Assist the individual in daily living skills and individual goals.
