Job summary

Chrysalis Host Home is seeking caring families willing to open their homes to an adult with disabilities. As a Host Home Provider, you will offer a safe, supportive living environment and advocate for daily living goals while working from home.

Responsibilities include transporting to medical appointments, attending activities, completing monthly paperwork, maintaining a clean home, and helping the individual develop skills. Prior caregiving experience preferred; background checks required.

Qualifications

• Must pass Criminal Background Check for all adults in the home.
• Must be 21 years or older.
• Experience providing support to individuals with disabilities preferred.
• Must meet home safety requirements.
• Complete pre-service training requirements.
• Valid driver's license and clean driving record.

Responsibilities

• Transporting, scheduling, and attending medical appointments with the individual.
• Transporting to and from activities as needed.
• Completing monthly paperwork.
• Maintaining a safe and clean environment for the individual.
• Assist the individual in daily living skills and individual goals.
